Call 1300 777 837 or complete the quick referral form at uds.com.au. A service navigator will phone you to confirm details and book an intake.
How do I prepare for my plan reassessment?
- Gather progress reports from every provider (Unify can request these for you).
- Check each support is linked to at least one plan goal.
- List any unmet needs or new goals.
- Bring quotes or assistive-tech trials if requesting new items.
Do I need a referral from a GP or specialist?
No, not for most services under the NDIS. Simply complete our online referral form and you’ll be on your way to receiving services. If any specific documentation is needed (e.g. for assistive tech), we’ll guide you through it.
How do I know if my supports are working?
Our team tracks progress towards your goals using outcome measures, therapy notes, and session reviews. We’ll share regular updates and can adjust the plan if your needs or goals change.
Can you help me get ready for my first NDIS plan (or a plan reassessment)?
Yes. Whether it’s your first plan or a reassessment, we can help you gather the right documentation, clarify your goals, and explain what supports might be best suited to you.
NDIS and funding

Unify delivers Allied Health (physio, OT, speech, psych, podiatry), Specialist Behaviour Support, Support Coordination, and Disability & Peer Support (community access, skill-building, SIL/STA).
Do I need an active NDIS plan?
Yes. If you’re still applying, we can guide you through eligibility and evidence gathering, but your plan must be approved before NDIS-funded therapy or support starts.
Which NDIS budget categories do your invoices use?
- Capacity Building, Improved Daily Living (allied health therapies)
- Capacity Building, Improved Relationships (behaviour support)
- Capacity Building, Support Coordination
- Core, Assistance with Daily Life / Social, Community and Civic Participation (disability & peer support)
How does Unify write strong NDIS reports?
Our clinicians use NDIA-aligned templates: clear functional impact statements, measurable goals, capacity-building outcomes and value-for-money reasoning. This helps reduce back-and-forth with planners.
Can you help with behaviour support plan (BSP) lodgement and restrictive-practice management?
Yes. Our practitioners write Interim and Comprehensive BSPs, lodge them through the NDIS Commission Portal, liaise with the NDIS Quality & Safeguards Commission, and coach your team on data collection and strategies to reduce restrictive practices.
How are travel charges calculated?
We follow the NDIS Pricing Arrangements. We’ll always try to pair you with a clinician local to you, so you pay less for travel and have more funding to use.
Are telehealth sessions funded the same as face-to-face?
Yes. Telehealth uses the same line items as in-person therapy, provided it meets participant goals and is clinically appropriate.
Do you support plan-managed, self-managed, and NDIA-managed participants?
Before booking, ask our team to confirm the service, practitioner, fees and payment arrangements for your funding type. For NDIA-managed supports, check the provider’s relevant registration scope. Read the NDIA provider guidance.
What makes Unify different from other NDIS providers?
We’re outcome-focused, not box-ticking. With NDIA-aligned documentation, dedicated service navigators, and a warm, human-centred approach, we focus on real impact, not just hours on a calendar.

What kind of experience do Unify clinicians have?
We take a tailored approach to allocating a clinician who suits your needs and presentation. Our clinicians have experience with a range of physical, mental health and psychiatric conditions, so we can match you with someone suited to your clinical needs.
Are Unify’s support workers experienced?
We recognise the need for you to have support workers that help you reach your goals. As a multidisciplinary provider, we go the extra step: our support workers are trained by our Allied Health and specialist clinicians.
Which locations do you cover?
We have teams on the Gold Coast and in Brisbane (QLD), Western Sydney (NSW), and Melbourne’s Metro North, West & East and Ballarat (VIC). Telehealth is available Australia-wide. See our locations to find out if we’re near you.
What’s the difference between support coordination and specialist support coordination?
Support Coordination helps you understand your plan, source providers and prepare for plan reassessments. Specialist Support Coordination is a higher level of support, used when there are complex barriers, multiple agencies or significant risk factors.
Do you offer after-hours or weekend services?
Our office is open Monday to Friday, 9am to 5pm. Some in-home and community supports may be available outside these hours by arrangement, so ask our team what's possible for you.
Can I choose my clinician or support worker?
Yes, absolutely. We’ll match you based on your goals, preferences, and cultural needs. If you ever feel like a change would suit you better, just let us know and we’ll support a smooth transition.
What if I don’t know what services I need yet?
That’s okay! Our intake team and support coordinators can help you explore your goals and funding so we can suggest the best service mix for your plan.
Can I receive service at your clinics?
Yes, we have a variety of clinics across Victoria, New South Wales and Queensland. Visit our location page to find your nearest Unify office.
What makes Unify different from other allied health or therapy providers?
We specialise in disability services, so our in-house team of experienced clinicians can deliver supports specific to your disability. We live and breathe disability services, from therapy to reports that clearly set out your support needs.

Do you support adolescents and adults?
Yes. We offer services across the lifespan, tailored to your stage of life.
Do you support mental health and psychosocial disability?
Absolutely. We have clinicians trained in working with psychosocial disabilities, and can provide both therapy and Specialist Support Coordination for participants with complex mental health needs.

Yes. Our services are mobile and flexible, whether at home, at school, at work or in your local community. We’ll work with you to deliver supports where they’re most effective.
